End of post report form for the Trust for those doctors who do not require an annual appraisal

If a Doctor is not due an appraisal whilst working for the Trust this 1 page exit report form should be completed by their relevant Medical Manager i.e. Clinical Lead, Education Supervisor. They should retain a copy as they will need to include it in their supporting evidence when their next annual appraisal is due.
